The survey will lead you through all the questions that are relevant for you. If you have experienced none of these things then please do not fill out the survey.
The survey is seeking information from people over the age of eighteen who live in Queensland. Therefore, if you are not a resident of Queensland, you will be blocked from completing this survey. However, we thank you for your interest.
After completing this survey you will be given the chance to enter a prize draw for an iPhone and to be considered for membership of some focus groups. However your personal contact details will not be linked to your answers to this survey.
